If this is the first time you have submitted a form to the IRB, please read all instructions prior to filling out the forms, including the updated information on Adobe at the bottom of this page. Again, please contact the IRB administrators at any time with questions.

FORMS UPDATES

4/17/08: The amendment submission form has been revised as noted in the email to IRB contacts dated 4/9/08 and 4/17/08. This form version is dated April 2008 and is effective immediately. The amendment form version dated APRIL 2007 will only be accepted for amendment submissions through April 22, 2008.

1/10/08: Supplemental form H has been revised to reflect updated language from the RADRAC. This form version is dated December 2007 and is effective immediately.

11/29/07: Supplemental form G has been revised to reflect a change in the requirements for submission of this form as outlined in the email sent to IRB contacts on 11/29/07. This form version is dated October 2007 and is effective for deadlines beginning in January 2008.

The IRB submission forms are available in Adobe (pdf) format only. Any form submitted to the IRB should be typed and, if a signature is required, should contain the original signature of the Principal Investigator. Note that Adobe Writer must be installed on your computer in order to save any information you type on a form

Please also note that if you are using a computer in the Medical Center, it is probable that Adobe Writer is not installed on that computer and you will not be able to save any changes you make to a form opened on that computer.

Please read the following prior to filling out forms:

1) If possible, the IRB forms should be accessed using Adobe version 6.0. The newest Adobe Writer/Reader programs include version 7.0 and 8.0. You will not be able to type your IRB forms correctly with these versions; however, you will still be able to open and type on the forms. If available, please open and type forms in version 6.0. If you are not able to use version 6.0, the IRB will accept forms written in higher versions.

2) If you wish to fill out the form online and print a hard copy (without saving what you have typed), then you only need Adobe Reader on your computer. (Available online for free at http://www.adobe.com/products/acrobat/main.html .) A hard copy should be retained for your files. 

3) If you wish to save the completed form, you must purchase the Adobe Writer program. Please contact your department regarding this issue. Once Writer is installed, for any form you wish to save the blank form must first be saved on your computer. Then all information typed thereafter may be saved. If you type any information before saving the blank form, the information will not be saved.

4) When printing the forms, there is an option on the print screen with a check box "shrink oversized pages to paper size" or a "Page scaling" option. Please turn this option off or check "none" before printing, as these forms have non-standard margins.
