Occupational Health & Safety (OHS)

Requirements for the Occupational Health & Safety Program 

Investigators and staff being listed on an IACUC animal care and use protocol (ACUP) for the first time must complete the following items.
  • Reviewing the Occupational Health & Safety Information Module for Animal Users.
  • Successfully completing the accompanying Quiz on Occupational Health and Safety.  This quiz needs to be successfully completed every three years.
  • Completing the Occupational Medicine Health Questionnaire for Animal Users. This questionnaire must be submitted to the University of Chicago Occupational Medicine office (UCOM) in L-156, MC 7103,  in a sealed envelope.  The health questionnaire is a one-time requirement.  However, if over the course of the research, one of the following statements becomes applicable, a review of the occupational health issues by UCOM is required.  Please call 2-6757 to schedule an appointment.
    • You have developed an illness or begun a new medical treatment that causes immune system compromise.
    • You are working with cats AND are pregnant or planning pregnancy.
    • You are working with sheep.
    • You have developed an allergy to your research animal or have developed asthma.

Directions if you have previously registered for OHS Informational Module/Quiz:
  1. New website is: http://www.aalaslearninglibrary.org/default.asp
  2. Log-in using the same username & password that was used to access ResearchTraining.org.  If you have previously registered but can't remember your username or password, please don't register again.  Contact one of the IACUC staff as they can inform you of this information.
  3. After logging in, place curser over 'Libraries' tab then click-on 'Animal Care and Use Courses'.
  4. Click-on 'Researcher Training'.
  5. Then, click-on 'Course' to review the informational module,or if you wish to take the quiz, click-on 'Exam'.
  6. After successfully completing the quiz, click to view the transcript and send it to the IACUC office by:
    1. Print-out and fax to 4-0659, or
    2. Save and email to: lenora@bsd.uchicago.edu

Directions if you have never registered:

  1. Go to new website at: https://www.aalaslearninglibrary.org/default.asp
  2. Click-on 'Enroll now'.
  3. Click-on the radio button to 'Join a group'
  4. The Access Code is: UofC1890
  5. Enter username, password and log-in.
  6. After logging in, place curser over 'Libraries' tab then click-on 'Animal Care and Use Courses'.
  7. Click-on 'Occupational Health and Safety for Animal Users'.
  8. Click-on 'Course' to review informational module; click-on 'Exam' to take the quiz.
  9. After successfully completing the quiz, click to view the transcript and send it to the IACUC office by:
    1. Print-out and fax to 4-0659, or
    2. Save and email to: lenora@bsd.uchicago.edu
